Bookstore Customer Service Survey

This is a sample of an online bookstore customer service survey that I can customize, load on my server, and provide to you as a link that you can distribute via email to as many customers as you wish.  The data will flow into my server, where it will be tabulated, graphed and reported back to you in a .pdf report that you can use to evaluate and change your service program. The data will roll out separately for the various categories of customers in question one.
