How Well Are Our Strategies and Activities
Aligned for Quality Performance?
Instructions
to respondent: You’ve been asked to give
your honest and objective feedback about your healthcare organization. Your responses will be managed so that they
stay completed anonymous. The purpose of
this survey is to help your organization identify its strengths that it can
continue to develop and any weaknesses that can be reversed to better meet the
needs of the patients you serve and the employees who make up the healthcare
team.
We
are hoping that this survey will provide valuable information that can
facilitate improvements within your organization. The administration of your healthcare
organization has asked that this survey be conducted so they can better
understand how the members of the healthcare team feel about various aspects of
the organization and to identify ways to make patient care delivery systems
better. This process recognizes that the
people who understand those systems best are those, like yourself, who have to
work within them every day.
For
each statement please click the box that best represents your feelings. Once you have completed the survey simply
click the submit button on the bottom.
The data will be anonymously tabulated into averages and graphs and a
final report will be created. Thank
you!
